They say you’re not really a New Yorker unless you own a nightguard! Research has shown that during episodes of grinding, our heart rate and pulse increases, which has a correlation between stress and grinding. Although yoga classes may help you find your Zen, it may not prevent you from damaging your pearly whites!
TMJ stands for temporal-mandibular joint, which is vital in the function and movement of the jaw. Complications in this joint may be caused by misalignment of the teeth, trauma, or excessive muscle tension. Problems in the TMJ can cause headaches, earaches, popping of the jaw, pain in the jaw muscles, and soreness in the area that may extend to the face. Dr. Plotnick has various methods for TMJ treatments, including professional nightguards.

WHAT TO EXPECT
2 appointments
YOUR FIRST APPOINTMENT
At your first appointment in Williamsburg, a TMJ assessment will be performed, checking your range of movement, feeling for tension in the muscles surrounding the jaw, and checking for clicking or crepitus (crackling noise) and nerve asymmetry. Since there are many TMJ appliances, careful assessment is necessary to ensure the right appliance is chosen for you. After your assessment, an impression of your teeth is taken with a putty-like material and sent to our laboratory technician to create a strong, durable appliance that lasts for many years.
YOUR SECOND APPOINTMENT
During your second appointment, we will have your new oral appliance ready for you. We will provide instructions on how to place the appliance, as well as take it out and clean it. We will also review the benefits of using the nightguard and the disadvantages of not using it. Be assured, Dr. Plotnick will have your TMJ therapy in your best interest, and following her instructions will minimize any symptoms you may be experiencing.

Frequently Asked Questions
How do night guards help with TMJ therapy?
Night guards, particularly custom-fitted ones, help with TMJ therapy by creating a protective barrier between teeth, which reduces jaw muscle tension, minimizes grinding, prevents tooth damage, and eases TMJ joint pressure while sleeping. They help reduce tension headaches, allow jaw muscles to relax, and can promote better jaw alignment.
How can I prevent TMJ?
Preventing TMJ (temporomandibular joint) disorders involves reducing strain on the jaw joint and muscles. Key strategies include avoiding hard/chewy foods, managing stress to stop teeth clenching, practicing good posture, and using a custom night guard if grinding occurs.
What are the symptoms of TMJ?
TMJ disorders commonly cause jaw pain, clicking or popping sounds, locking of the jaw, and difficulty chewing. Symptoms can also include headaches, earaches, facial pain, and neck or shoulder tension.
How long do I need to wear a night guard to help with my TMJ disorder?
For TMJ disorders, you should typically wear a night guard every night for 4 to 8 weeks to experience significant improvements in symptoms like jaw pain and headaches. While some patients feel relief in days, others require 6 to 12 months for long-term stabilization, with many continuing to use their night guard to manage, prevent, and protect teeth from long-term wear.
What’s the difference between store-bought and custom night guards?
Custom night guards are made by dentists from impressions of your teeth and offer a precise fit, superior comfort, and durable protection against teeth grinding. They can last for years. On the other hand, store-bought guards are "one-size-fits-all" or "boil-and-bite" options that are cheaper but often bulky, uncomfortable, and less effective at preventing damage.
How long will it take before I notice an improvement in my TMJ?
Most people notice initial improvements in TMJ symptoms within 1 to 3 weeks of consistent nightly use of a custom night guard. While some may feel relief in just a few days, a significant, long-term reduction in pain, such as fewer headaches and less jaw soreness, often takes 4 to 6 weeks. It can differ from patient to patient.
Should I wear the night guard every night?
Yes, you should wear your night guard every night for teeth grinding or TMJ issues, as it is designed for consistent, nightly use to prevent enamel wear, tooth fractures, and jaw pain. That said, Dr. Plotnick will go over your customized treatment plan to ensure you know exactly when to wear your night guard and for how long.
Are night guards for TMJ therapy covered by insurance?
Night guards for TMJ therapy are often covered by dental insurance, provided they are deemed medically necessary. In this case, since the guard is being used to treat a dental and medical condition, most of the cost should be covered. That said, it’s important to check with your individual insurance provider.
What’s the difference between TMJ and TMD?
TMJ refers to the temporomandibular joint itself or the hinge connecting the jaw to the skull. TMD (Temporomandibular Disorders), on the other hand, refers to the inflammation, pain, or dysfunction of that joint and surrounding muscles. People often use TMJ to describe the pain, but the disorder is actually TMD.
Can TMJ go away on its own?
TMJ disorders can go away on their own, especially if the condition is caused by temporary stress, minor injury, or habits like grinding. However, chronic cases often require treatment, such as night guards, physical therapy, or lifestyle changes. If symptoms are mild and infrequent, they may resolve within a few weeks.
